Athletic Training

The Athletic Training staff is a vital part of each Argonaut squad. Our group of Certified Athletic Trainers provides regular treatment for both major and minor injuries, while also providing preventative care.

Argonaut student-athletes also enjoy the best in medical care thanks to UWF’s partnership with world-renowned surgeon Dr. James Andrews and his team at the Andrews Institute. When injuries sideline the Argonauts, the worldclass team of doctors at the Andrews Institute ensures that they receive quality medical care and are back on the field as soon as possible.

Dear Student-Athletes:

Please access the UWF Athletic Training Department's link on the University of West Florida's Athletic website at www.GoArgos.com. to print and complete the required medical forms. The following is a checklist of the required medical forms that must be completed and on file in the UWF Athletic Training Room before you may participate in intercollegiate athletic practices or competitions at the University of West Florida:

Personal Information/Insurance Information (Must include parent signature if covered under parents insurance)
Medical Consent and Release Packet
- Copy of all current insurance card (front and back) and pharmacy & dental insurance cards (if available)
Medical History Packet (New student athletes only)
Athletic Training Policies
Lightning Policy
Drug Testing Policy
Emergency Action Plan

Once again, physicals will be offered on a specified date and time free of charge. Please make it a priority to stop by the athletic training room as soon as you arrive at UWF. All new athletes must receive a physical before athletics participation is permitted. For returning student-athletes, a physical recheck will be performed by the UWF certified athletic trainers and the outcome of that recheck will determine if you need a new physical. Returning student-athletes will also have the opportunity to see a physician if they so wish during physicals. There will also be other medical paperwork that must be completed before you are cleared to participate with your team.

Please make sure all items are complete and all necessary signatures are provided. If you are unable to or do not have the capabilities to print out any of these forms please contact the athletic training room at (850) 474-2985 and we will gladly mail you hard copies of the forms for you to complete.

Your cooperation in returning the required paperwork is greatly appreciated. Please contact us with any questions or concerns. Also enclosed is a list of NCAA banned substances that the NCAA and the UWF Athletic Department will be testing for during your tenure at UWF. The UWF Athletics Drug Testing Policy will be reviewed with you at a later date on campus.
